YOUR RIGHTS & RESPONSIBILITIES BOOKLET
This booklet will describe your mover's responsibilities to you and your rights as a customer. It will also outline your responsibilities as the customer and many of the movers policies.

AMSA ARBITRATION PROGRAM INFORMATION
In the unlikely event a dispute arises which can not be resolved satisfactorily, the American Moving & Storage Assocation has put in place an arbitration program to resolve these issues. This booklet describes the program.

HIGH VALUE INVENTORY
Customers are required to declare to the mover any items to be moved which have a value of $100 per pound or greater. For example, a $10,000 painting which weighs 20 pounds should be declared since the $10,000 value is worth more than the weight times $100 (20 pounds times $100 equals $2,000). But
a TV worth $4,000, which weighs 50 pounds, would not to be declared since the value is less than the weight times $100 (50 pounds times $100 equals $5,000). This form is used to declare this information. Instructions are included. Additional copies can be used if a sheet if filled up.
